Coordination of inpatient and emergency room care if hospitilized

02 Review Membership Agreement
We will go over our membership agreement and intake forms during our initial meet and greet. Once the agreement is signed we will create a patient chart in our electronic medical record (EMR) and can start addressing urgent medical concerns.
03 Medical Records Collection
We will work to consolidate all your medical records into our EMR. We use Elation as our EMR which integrates with most major hospital systems including Cerner and Epic as well as outpatient lab and imaging centers - making this process as smooth as possible.
04 First Appointment
A comprehensive 1:1 appointment with your physician discussing your medical history, treatment plans with sub-specialists and both short and long term healthcare goals.
